Dexrazoxane for Injection


DEXTIV for Injection is a cytoprotective agent primarily used to protect against the cardiotoxic effects of anthracycline chemotherapy and to treat extravasation (leakage) of anthracycline drugs into surrounding tissues. Here are the main uses of Dexrazoxane:


 1. DEXTIV in Anthracycline Chemotherapy

   - Prevention of Cardiomyopathy: Dexrazoxane is used to prevent cardiotoxicity (heart damage) in patients receiving anthracycline chemotherapy, such as doxorubicin or epirubicin, which are known to cause dose-dependent heart damage. It is typically administered to patients who are receiving high cumulative doses of these drugs or who are at increased risk for developing heart complications.

   - Breast Cancer and Other Cancers: It is often used in patients with metastatic breast cancer and other cancers where anthracyclines are part of the treatment regimen.


 2. Treatment of Anthracycline Extravasation

   - Management of Tissue Damage: Dexrazoxane is also used to treat anthracycline extravasation, a condition where the chemotherapy drug accidentally leaks from the vein into surrounding tissues, causing severe tissue damage. Dexrazoxane works by chelating iron, which is necessary for the formation of free radicals that cause tissue damage. It helps to minimize the extent of injury and improve healing.


Mechanism of Action

Dexrazoxane is a prodrug that, once converted to its active form in the body, works by inhibiting topoisomerase II, an enzyme involved in DNA replication. This inhibition reduces the formation of anthracycline-induced free radicals that are responsible for cardiotoxicity. Additionally, by chelating iron, it reduces the production of damaging free radicals in the heart and other tissues.


Important Considerations

- Side Effects: Common side effects include nausea, vomiting, diarrhea, low blood cell counts (myelosuppression), and elevated liver enzymes. Serious side effects can include secondary malignancies (such as leukemia) and severe infections due to immunosuppression.

  

- Administration: Dexrazoxane is administered intravenously, typically 30 minutes before the administration of anthracycline chemotherapy to provide cardioprotection. For treating extravasation, it is given as soon as possible after the occurrence, typically in a series of three daily infusions.


- Monitoring: Regular monitoring of cardiac function, complete blood counts, and liver function tests is recommended during treatment with Dexrazoxane. In patients receiving it for cardioprotection, baseline and periodic cardiac evaluations (e.g., echocardiograms) are important.


- Contraindications and Cautions: Dexrazoxane is contraindicated in patients who are allergic to it and should be used cautiously in those with pre-existing bone marrow suppression or liver disease. It should not be used simultaneously with other topoisomerase II inhibitors due to potential additive effects.


- Drug Interactions: There are potential interactions with other chemotherapy agents, and careful consideration is needed when combining Dexrazoxane with other treatments that have similar toxicities.

- Reduction of Cardiotoxicity: Dexrazoxane significantly reduces the risk of heart damage in patients undergoing anthracycline chemotherapy, allowing them to receive higher cumulative doses of these effective cancer treatments.
